In our practice of emergency medicine, we evaluate kidney function every day of our career. However, far too often do we simplify it to only a creatinine and GFR value. Here I will break down how we can better care for our patients by understanding the utility of urine electrolytes, renal tubular acidosis, and other tips for managing kidney related issues.

Dialysis forms the core of end stage renal disease management--a fact many of us encounter all too frequently in our work. Whether peritoneal or hemodialysis, bypassing the kidney creates a wealth of opportunity for things to go wrong. Among the possibilities are steal syndrome, peritonitis, bacteremia, catheter dysfunction, hemorrhage, and more. Further, innovations such as the HeroGraft can improve the lives of our patients but may be foreign to us as emergency physicians.

A constant battle many of us face is the contention between a patient's eGFR and the utility of a contrasted CT scan. Once upon a time, it was thought that contrast was the likely culprit in many admitted patient's subsequent elevated creatinine levels. Now, the pendulum has swung and many emergency physicians scoff at the idea of contrast induced nephropathy. But what does the literature say? Is there evidence that shows we should or should not avoid iodinated contrast in our patients?
